1. Create an Account

   There are three ways to join: You can visit the enrollment website at JoinAllofUs.org, download and use the All of Us app, or call us at (877) 268-2684 to visit an enrollment location (see below for a list of locations).

2. Give Your Consent

   You will be asked to watch a series of videos, which provides detailed information about the program. Once you feel comfortable, you can electronically sign consent forms including: a consent that gives All of Us access to your electronic health records (EHR), a consent for you to receive your DNA results.

3. Answer Health Surveys

   Answer the health surveys available on your participant portal dashboard: The Basics, Overall Health, Lifestyle.

4. Schedule your Enrollment Visit

5. Physical Measurement and Biospecimen Collection

   During your appointment, we will take your physical measurements, which could include your blood pressure and heart rate, height, weight, and waist and hip circumference. We will also collect a blood and urine sample. Thank you for you time! Good news: You will be compensated $25 for completing the visit.
